Insurance coverage is an important part of the referral process. Our team can help confirm whether a patient's plan is accepted and review applicable coverage requirements.
Accepted insurance carriers: List to be confirmed with APLMED HC before publication.
Plan participation, authorization requirements and patient eligibility must be verified individually. We will not display unconfirmed insurer names.
